Job Title: Youth Worker

Leduc LINX Connect Centre provides a variety of services to individuals with developmental disabilities.
Job Summary:
· Responsible for advocating for clients to access resources.
· Responsible for assessing the needs of clients to address challenges and to assist them in developing life and social skills.
· Providing support to foster independence.
· Assessing the needs of the client and identifying goals and plans with the client
· Helping clients to gain access to other community resources and activities.
· Building social and other support networks to assist in clients' independence, autonomy, and personal development as appropriate.
· Facilitating practical changes and/or initiate activities to enhance in the clients' physical and social environments to promote their well-being and self-esteem.
· Providing youth with problem-solving techniques, communication, and conflict resolution through modeling and coaching.
· Crisis intervention; using Mandt training and access to resources.
· Attending all team meetings, ISP?s and debriefs when required.
· Maintaining accurate daily documentation of client?s activities (Daily notes).
· Documenting and reporting to Team Lead and management any incidents that are of concern or may cause harm to clients, staff or others (Incident reports).
· Writing and maintaining behavioral documentation (ABC charts).
· Responsible for maintaining ongoing communication with LINX management.
· Ability to provide learning opportunities and behavioural support.
· Implement and utilize person-centered, trauma informed plans and programs.
· Facilitate inclusion, and support models designed to support and assist participants in an autonomous role.
· Maintain timely, accurate and professional documents in accordance with LINX policies and procedures.
· Transporting clients within the City of Leduc as necessary.
· Supporting clients in activities of interest (varies by client).
· Responsible for engagement and interaction with clients.
· Acting as a representative for LINX in the community.
· Other duties as required.
