<p><strong>Title: </strong><strong>Nurse Practitioner</strong></p><p><strong>Employment Terms:</strong> Regular<strong>, </strong>Full -Time</p><p><strong>Schedule: </strong>35 hours/week. Monday, Wednesday, Thursday, Friday: 9 am to 5 pm. Tuesday: 12 noon ? 8 PM. One Saturday per month 9 am ? 2 pm (with corresponding time off during the week as lieu). It is an innovative partnership between Flemingdon Health Centre (FHC) and TNO-The Neighbourhood Organization (TNO) funded by the Ministry of Health. HATP is a community driven initiative aimed to enhance interdisciplinary comprehensive primary health care, improve access to health, social and community services in Thorncliffe Park. <strong>This is an HATP-FHC position.</strong></p><p><br> <strong>Position Summary:</strong><br> The Nurse Practitioner will work as an integral part of our interdisciplinary team to provide comprehensive primary health care services. As an independent professional the Nurse Practitioner will work to full scope of practice to contribute to clients? health providing assessment, prevention, education, rehabilitation and therapeutic support services consistent with the standards of care outlined by the College of Nurses. We are seeking a Nurse Practitioner who is committed to community based, client-centred care, and passionate about working collaboratively to address the social determinants of health, to reduce barriers and improve access to health care for the clients across all sites.</p><p><br> <strong>Qualification, Skills and Proficiencies:</strong></p><ul><li>Registered Nurse with the College of Nurses of Ontario in the Extended Class.</li><li>Baccalaureate in Nursing and graduate of recognized Educational Program for Primary Care Nurse Practitioners</li><li>2-3 years of experience in primary care or community nursing will be an asset.</li><li>An ability to work as a member of an interdisciplinary health care team, responsible and accountable for own practice</li><li>Demonstrated experience in chronic disease management and prevention, and a good understanding of the role of health promotion</li><li>Experience working with marginalized communities and complex clients</li><li>Commitment to anti-racism and anti-oppression practices</li><li>Excellent oral and written communication skills</li><li>Ability to work flexible hours, evening and weekend work expected in alignment with HATP?s Strategic Priority of <em>Catalyzing Community Driven Care</em> and increasing access to health services and supports for equity deserving community members</li><li>Ability to speak other languages, preferably Urdu, Slovak, or Arabic is an asset</li><li>Knowledge of EMR System an asset (Practice Solutions)</li><li>Non-judgmental/non-confrontational attitude</li><li>Vaccines (COVID-19 and others) are a requirement of the job pursuant to the Ontario Human Rights Code</li></ul><p><strong>Responsibilities include:
