Under the direction of the Programs and Volunteer Manager, this position will be responsible for providing Resident management through all stages of assessment, treatment, consultation and follow-up services to Residents. The successful candidate will be familiar working in a team environment and in delegating treatments to qualified Physiotherapy Support Person(s) (PSP). Performance of the program will provide the physical and emotional requirements necessary to effectively provide and maintain a high quality of care for our Residents, in a manner that is consistent with GROVE PARK HOME?S Mission, Service Philosophy, the policies and procedures and other applicable regulations and guidelines.
*Qualifications:*
* Graduate of a recognized University Physiotherapy degree program or graduate of a Post-Secondary program in Physiotherapy.
* License to practice with the Ontario College of Physiotherapists.
* In good standing with the College of Physiotherapists of Ontario
* Eligible for membership in the Canadian Physiotherapy Association.
* Experience in geriatric, rehabilitation or Long Term Care an asset
* Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively with PSP?s as well as with an interdisciplinary rehabilitation team
* Effective interpersonal, organizational and communication skills
* Effective verbal and written communication skills
* Computer skills including experience with Point Click Care, an asset
* Excellent interpersonal skills and problem solving
*Responsibilities:*
* Conducting assessments
* Formulating goals and recommendations, follow up plans, evaluations
* Planning and implementing with PSP person centered, goal-directed treatment programs
* Providing education to Residents, families and team members
