*Role Specification:* * Permanent * Full-Time (across 2 LTC homes) * Independent contractor *Minimum Requirements:* * Completion of recognized Nurse Practitioner program * Current extended class certification with College of Nurses of Ontario * Experience with chronic disease management and geriatric care * Current vulnerable sector police check * COVID vaccination * New grads are welcome *Rewards/Perks:* * Health benefits * Autonomy and independence * Educational days (Quarterly) * Above average salaries *Job Responsibilities:* * Responsibility over medical care delivery to LTC residents * Management of stable complex chronic co-morbidities in collaboration with MDs * Collaboration with an interdisciplinary team in the course of care delivery * Provide high quality customer service * Provide clinical / medical education to clients and staff * Participate in care conferencing * Role is about 70% clinical and 30% quality improvement, education, and leadership *Company Profile:* * Can-NP-Serv is an NP led organization that focuses on delivering clinical value to customers, while elevating the NP role in the context of higher wages, autonomy, full scope practice and improved overall conditions and terms.
