We value health equity, inclusion, community engagement, accountability & transparency, excellence and collaboration & partnerships.<strong> </strong></p><p></p><p><strong>Job Summary:</strong></p><p>The Registered Nurse (RN) will be integral in delivering comprehensive diabetes management and prevention services, with an emphasis on both individual care and community engagement. The RN will provide education, conduct assessments, and support individuals living with diabetes, prediabetes, and those at risk for diabetes. This role involves collaboration with an interdisciplinary team to optimize diabetes care and contribute to the overarching goals of chronic disease prevention and management.</p><p><strong>Key Responsibilities:</strong></p><ul><li>Client Assessment and Education: Conduct thorough assessments of clients? knowledge, readiness to change, and socio-economic backgrounds. Provide one-on-one education on diabetes management and prevention, incorporating cultural considerations.</li><li>Group Education: Plan, deliver, and evaluate diabetes education and self-management group programs. Ensure programs are culturally relevant and address the needs of diverse populations.</li><li>Advance Health Equity: Create educational materials and resources tailored to the needs of South Asians and other diverse groups. Work to ensure equitable access and outcomes for all clients, including underserved and diverse communities. Maintain diabetes samples and supplies for distribution to equity-deserving groups as needed.</li><li>Community Engagement: Participate in community outreach initiatives to promote diabetes prevention and self-management. Engage with local organizations and healthcare providers to expand the program?s reach and effectiveness.</li><li>Risk Level Assessment: Utilize program-specific tools to assess client risk levels
