The Camrose Primary Care Network (PCN) seeks a driven, experienced, and skilled Nurse Practitioner to provide patient centered care to full scope of practice, with a focus on vulnerable populations (such as but not limited to youth mental health and women?s health), complex patients and those without connection to a primary care provider/family physician. The NP will be a key person in supporting the Patient?s Medical Home model of care at CPCN.
This is your opportunity to work alongside a multi-disciplinary team made up of nurses, mental health professionals, physicians, and allied healthcare providers, to positively impact lives through outcome driven, evidence-based care. In addition, your high level of confidence and competency is essential when collaborating with external stakeholders to support sustainable process changes to meet the needs of all residents in our community. This position may be based from a variety of community settings such as Physician clinics, PCN offices and other local community agencies. Hours of work may include weekdays, evenings and/or occasional weekends.
A Nurse Practitioner with current and active registration, enrolled with CARNA extended practice roster, and must be licensed to work with family/all ages of patient populations. The ideal candidate will have a minimum of three (3) years? experience in a community setting with a focus on primary care services, with the ability to work in a fast-paced environment requiring flexibility, balancing of competing priorities and a willingness to work collaboratively. The capacity to develop new skills and apply theory into practice is valued in your aptitude for aiding in program development. The role requires a constructive, empathetic, and respectful approach to interactions with both patients and colleagues.
*Formal Education and Additional Certifications:*
· Master?s degree in nursing, with Nurse Practitioner certification.
· Full licensed and in good standing with CARNA.
· Current BCLS certification for health care providers.
· Women?s health an asset.
· Non-violent crisis intervention training an asset.
* Must have a recent (within the past three months) criminal record check/police information check (including vulnerable sector search) prior to the first day of hire.
* Valid driver?s license and access to reliable vehicle required.
*Experience and Skills Required:*
* 3 or more years of nursing experience, preferably in a rural primary care setting.
* Experience of working in a walk-in medical clinic, as well as women?s health is an asset.
* Experience with various vulnerable populations
* Experience working within a multidisciplinary care team model.
* An understanding of Harm Reduction, Population Health principles and health behaviour change methods (e.g., Motivational Interviewing). Comfortable working with Vulnerable populations in a variety of settings including but not limited to less traditional health care environments.
* Knowledge of and interest in principles of a population-needs based approach, primary care, primary health care and the medical home model is an asset.
* Must have excellent working knowledge of Microsoft Office products and experience with electronic medical record (EMR) documentation.
* Awareness of routes to access health resources for at risk and vulnerable populations.
* Ability to be self-directed member of a multidisciplinary team who provides holistic patient care based on advanced clinical knowledge and skills using independent and collaborative problem solving and decision-making techniques.
* Demonstrates good organizational skills in coordinating plan of care for individuals with other team members.
* Ability to follow and adhere to Camrose Primary Care Network procedures, guidelines, goals, and objectives, and confidentiality policies.
