*Position Summary*
Dan?s Legacy is currently seeking 2 full-time, trauma-informed, and culturally sensitive Registered Social Workers (RSW) to work with youth at risk, ages 15 to 25 years old, in the Vancouver/Burnaby/New West Area. Our Social Workers are a part of an integrated team that is linked to our core Clinical Counselling Program and our Cook Training Program. This position reports directly to the Outreach and Social Worker Supervisor and the Clinical Counselling Manager.
*Duties & Responsibilities: *
· Daily outreach responsibilities across the Lower Mainland including Vancouver, New Westminster and Burnaby
o Example: completing applications such as Income assistance, Person with Disabilities, Person with Multiple Barriers, BC housing, etc.
· Develop a psychosocial assessment and treatment plan by reviewing the referral, collecting information on the client?s social, emotional, cultural background, their response to illness, coping mechanisms, and assessing their environmental and financial conditions.
· As part of the interdisciplinary team with Therapist and Outreach Workers, shares insights about the client's psychosocial situation and contributes to creating a comprehensive care plan in collaboration with the client
· Help clients with basic needs such as obtaining food security, shelter resources, and medical services
· Liaise with community partners and other organizations to help our clients access other supportive programming
· Represent and take part of city situational tables and as well as any other community meeting related to social service resources
· Collaborate with government organizations, such as the Ministry of Children and Family Development, to ensure clients? health and safety
· Act as a liaison between clients and external agencies, ensuring that client voices are heard and that their concerns are addressed in policy discussions and service delivery plans.
· Assist client during emergencies by creating safety plans, assessing duty to report, and actively acting on the duty when identified
· Accurately upload information relating to the day-to-day clinical activities, such as session notes and attendance, with clients in compliance with the organization?s adopted data system (ShareVision) and its reporting requirements
· Complete case management, such as scheduling and discharging clients, and making internal/external referrals, according to ethical and professional standards to support interdisciplinary functioning and consistent approaches to clients
· Attend weekly supervision with Outreach and Social Work Supervisor and/or Clinical Counselling Manager to improve skills, ensure provision of best care and, stay up to date with the latest techniques and ethical standards
· Participate in weekly team meetings with the outreach team and supervisor to collaborate on client cases, share insights, and exchange resources to enhance service delivery and support client outcomes.
· Proactively coordinate and conduct case discussion and treatment plan coordination with Clinical Counsellor at the Dan?s Legacy office or partner sites
· Support and collaborate with supervisor and manager on capacity development and training on social work-related topics
· Actively reach out to supervisor and/or manager for ethical decision making
· Comply with the code of ethic from the BC College of Social Workers
· Performs other duties as assigned
*Qualifications & Requirements:*
· A successful criminal record and vulnerable sector check will be required prior to the start of employment
· Valid BC Driver?s License and a dependable vehicle
· One year experience working with vulnerable youth is preferred
· An understanding and experience working in a trauma informed environment is preferred
· Bachelor?s degree in social work required
· A valid registration with the BC College of Social Workers required
