We value health equity, inclusion, community engagement, accountability & transparency, excellence and collaboration & partnerships.</p><p><strong> </strong></p><p><strong>Position Summary: </strong></p><p>The Nurse Practitioner will work as an integral part of our interdisciplinary team to provide comprehensive primary health care services. As an independent professional, the Nurse Practitioner will work to full scope of practice to contribute to clients? health providing assessment, prevention, education, rehabilitation, and therapeutic support services consistent with the standards of care outlined by the College of Nurses. We are seeking a Nurse Practitioner who is committed to community-based, client-centered care, and passionate about working collaboratively to address the social determinants of health, to reduce barriers and improve access to health care for the clients across all sites.</p><p><strong> </strong></p><p><strong> Responsibilities Include: </strong></p><p><span> </span><strong>Direct Client Care:</strong></p><ul><li>Provide comprehensive primary health care including assessments, diagnosis, treatment plans, follow up, referral, education and referral within the scope of practice of a registered nurse in extended class</li><li>Maintain accurate client visits and findings on current record system</li><li>Coordinate client care within FHC and/or HATP and with other health care agencies and specialists</li><li>Determine the need for and orders and interpret screening and diagnostic laboratory tests, chest and limb x-rays, diagnostic ultrasounds etc.</li><li>Counsel and provide information and follow up on abnormal results.</li><li>Prescribe designated drugs as specified by the CNO</li><li>Participate in Quality Improvement Programs.</li></ul><p><strong>Health Promotion and Education:</strong></p><ul><li>Implement health teaching and promotion with clients as part of clinical assessments and client care</li><li>Participate in identification of priority groups or targeted populations for speci
