MyoHealth Rehab and Wellness Centre has an exciting opportunity for a *Social Worker* to join our team of health care providers. We are a private clinic that specializes in the treatment of clients injured as a result of motor vehicle accidents. Joining this multi-disciplinary rehabilitation clinic in *Scarborough* offers the suitable individual the opportunity to collaborate with a team to ensure the provision of exceptional care to our clients in a culture that is client-focused, diverse, inclusive and encourages personal growth.
*Roles and Responsibilities:*
* Provide clinical consultation, psychosocial assessment, crisis intervention and counselling to patients and their families
* Preparation and documentation of assessments, treatment plans and patient clinical progress reports
* Collaborate with the social work team in providing comprehensive patient care including linkage to community resources; consultation with team members to ensure quality of patient care
* Liaise with colleagues, clinical staff, management, and external providers including insurance adjusters, lawyers and other members of the community
* Provide resource navigation within health care services and the community for patients and/or their families
* Act as a communication conduit between the patient/family and the clinical team
* Assist in the development and implementation of policies, priorities, and procedures to streamline day to day activities
* Commit to professional learning development activities as requested by Supervisor and/or the Clinic Director in a manner that is relevant to the role
* Engage in other responsibilities as may be assigned by the Supervisor and/or the Clinic Director
*Qualifications and Experience:*
* Master of Social Work (MSW) obtained from an accredited school of Social Work
* Current member in good standing with the Ontario College of Social Workers and Social Service Workers (OCCSWSSW) for practice in all applicable clinical areas
* Two years of experience working as an MSW in a health care setting/private clinic
* Experience with insurance procedures, trauma population and MVA is a strong asset
* Strong assessment, clinical interviewing and problem solving skills
* Excellent organizational and communication skills (oral and written), negotiation, mediation, and time management skills
* Demonstrated ability to work independently and collaboratively in an inter-professional team environment
* Knowledge of the health care system and community resources is an asset.
* Superior ability to manage multiple competing demands, set priorities in a highly dynamic and fast paced environment
* Computer skills required; ability to manage electronic documentation, workload and meet submission deadlines
* Sound decision making skills, judgement, professionalism and adherence to strict confidentiality policies
* Demonstrates cross cultural awareness and sensitivity
* Performance-oriented
* A valid driver's license is a strong asset
