*Job Summary*
The Nurse Practitioner is responsible and accountable for the comprehensive assessment of residents including diagnosing diseases, disorders and conditions. The Nurse Practitioner will work at Maplewood House, a long-term care home for 78 residents located in Abbotsford, BC. Our residents most often include older individuals, those living with frailty, aging-related physical and cognitive changes, dementia and other cognitive impairments, and chronic illnesses.
Maplewood Care Society (MCS) is a non-profit, charitable organization located in Abbotsford, BC and has been proudly providing housing and care services to older people in our community for 50 years.
*As a Nurse Practitioner you will:*
Be responsible and accountable for the comprehensive assessment of patients and clients including diagnosing diseases, disorders, and conditions.
Initiate treatment including health care management, therapeutic interventions and prescribes medications in accordance with the statutory and regulatory standards, limits and conditions, and employer policies and procedures.
Provide professional guidance to other health professionals and participates autonomously as a member of the interdisciplinary health team.
Collaborate with clients and other health professionals to identify and assess trends and patterns that have implications for clients, families, and communities; develops and implements population and evidence-based strategies to improve health services and practices.
Ability to self-assess performance and assume responsibility and accountability for own professional development, educational or consultative assistance when appropriate.
Reports and collaborates with the Chief Clinical Officer and has key accountabilities for quality assurance and patient safety.
*Qualifications*
Education & Experience
Current registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM) plus a minimum of 1 year of recent, and relevant clinical nursing experience.
*Key Responsibilities*
*1. Clinical Care and Assessment*
a. Conduct comprehensive health assessments, including medical history, physical examination, and diagnostic testing.
b. Develop and implement individualized care plans, including interventions for acute and chronic health issues.
c. Monitor, evaluate and adjust care plans based on residents? progress and changing health status.
d. Prescribe medications and treatments in accordance with BC regulations and within the NP scope of practice.
*2. Collaboration and Communication*
a. Collaborate with physicians, health care team members, and families to ensure continuity and coordination of care.
b. Serve as a primary resource for clinical advice and best practices to other team members.
c. Participate in family conferences to discuss care plans, provide education, and address any questions or concerns.
d. Communicate effectively with residents and their families to foster trust and understanding of care plans.
*3. Health Education and Promotion*
a. Educate residents, families and caregivers on health maintenance, chronic disease management and preventive health measures.
b. Conduct or participate in health promotion activities tailored to the LTC setting, such as fall prevention, wound care, polypharmacy, advance care planning, and infection control.
*4. Quality improvement and Compliance*
a. Engage in continuous quality improvement initiatives, helping to develop and implement policies and procedures that align with LTC standards and regulatory requirements.
