*Job Summary* Ontario Health Clinics Brantford FHO is seeking a full-time, permanent Primary Care Nurse Practitioner to join our group family practice. *Responsibilities* * Provide Primary Health Care to patients; assess, diagnose and evaluate acute and chronic illness, prescribe treatments, medications, diagnostics and follow up care. * Perform PAP tests, well baby care and administer immunizations. * Prepare, implement, review and evaluate the patient's plan of care. * Provide health teaching, and promote healthy life styles by incorporating a teaching plan with interventions and referring patients as appropriate. * Electronic documentation of health records using an EMR (previous Oscar/Avaros experience is a bonus). *Qualifications* * Must possess a Nurse Practitioner Certification from a recognized program. * Must possess a current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class or RN (EC), in good standing. * Must maintain competencies as per the governing College standards. * Must hold a current membership with the Registered Nurses Association of Ontario (RNAO). * Must possess proven leadership abilities and willingness to work in collaboration with other health care providers. * Must demonstrate excellent written and oral communication, critical thinking, interpersonal, care coordination and time management skills. * Must provide comprehensive care in a patient-focused manner. * Must be able to maintain comprehensive documentation based on the standards outlined by the College of Nurses of Ontario. * Previous experience in Primary Care is strongly preferred. *Job Type:* * Full-time * Permanent *Expected Hours:* * 9am - 4pm *Schedule:* * Day shift, four days per week * Tuesday to Friday (preferred but flexible) *Benefits:* * Flexible schedule * On-site parking * Paid sick time * Attractive vacation package * Continuing education stipend Join us in making a difference in the lives of our patients through dedicated, compassionate care!
