Job Description SOCIAL WORKER- Extendicare Scarborough Candidates are invited to apply for the position as a Social Worker, located at our 151-bed Extendicare Scarborough Home in Scarborough, Ontario. Reporting to the Administrator, the Social Worker works in collaboration with an interdisciplinary team to advocate and provide support and services to residents, families and staff. RESPONSIBILITIES • Completes resident pre-admission, admission and ongoing social work assessment to identify goals and strategies, expected outcomes for care and target dates. • Participates in interdisciplinary team conferences as required • Acts as liaison, as appropriate, with families. • Provides counselling or group sessions for residents and families as identified - i.e., orientation adjustment programs, crisis problem solving, bereavement counselling, etc. • Acts as a resource- LocationScarborough, ON
